Please read these Terms and Conditions carefully before using our services, website, or patient portal.

Acceptance of Terms

By accessing or using Alam Medical's website, patient portal, or services, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, you must not use our services.

Important: These Terms and Conditions govern your use of our digital services and supplement our Practice Policies provided during patient registration. For medical emergencies, please call 911 or visit your nearest emergency room.

Medical Services

Scope of Services

Alam Medical provides primary care services including but not limited to:

  • Preventive care and annual physicals
  • Chronic disease management
  • Acute illness treatment
  • Health screenings and vaccinations
  • Referrals to specialists when needed

After-Hours Care

For urgent medical concerns outside of regular business hours, please call our main line at (347) 509-4470 to reach our on-call service. For life-threatening emergencies, call 911 immediately.

Patient Portal

Account Registration

To access our patient portal, you must register for an account using the credentials provided during your first visit. You are responsible for maintaining the confidentiality of your login credentials and all activities that occur under your account.

Portal Services

Through our patient portal, you may:

  • View test results and health records
  • Request prescription refills
  • Send secure messages to your care team
  • Schedule and manage appointments

Response Time: While we strive to respond to portal messages within 2 business days, urgent medical concerns should be addressed by phone or in-person visit.

Appointments & Scheduling

Cancellation Policy

We require at least 24 hours notice for appointment cancellations. Late cancellations or no-shows may be subject to a fee:

  • Established patients: $50 fee for missed appointments
  • New patients: $75 fee for missed initial consultations

Chronic No-Show Policy: Patients with three or more missed appointments may be discharged from the practice.

Late Arrivals

Patients arriving more than 15 minutes late may be asked to reschedule to avoid disrupting other patients' scheduled appointments.

Billing & Payments

Insurance Processing

We participate with most major insurance plans. It is your responsibility to provide current insurance information at each visit and understand your plan's benefits, copayments, and deductibles.

Self-Pay Patients

For patients without insurance or with out-of-network benefits:

  • Payment is due at the time of service
  • We offer discounted self-pay rates for common services

Intellectual Property

All content on the Alam Medical website, including text, graphics, logos, images, and software, is the property of Alam Medical and protected by copyright and intellectual property laws.

You may not reproduce, distribute, or modify any content without written permission, or use our name, logo, or branding for any commercial purpose.

Limitation of Liability

Important Limitations

To the fullest extent permitted by law, Alam Medical shall not be liable for:

  • Any indirect, incidental, or consequential damages
  • Service interruptions or technical issues with our website or portal
  • Unauthorized access to your information due to security breaches beyond our reasonable control

Indemnification

You agree to indemnify and hold harmless Alam Medical, its physicians, and staff from any claims, damages, or expenses arising from your use of our services or violation of these Terms and Conditions.

Termination

By Practice

We reserve the right to terminate the physician-patient relationship for reasons including but not limited to:

  • Non-compliance with treatment recommendations
  • Abusive behavior toward staff or other patients
  • Repeated missed appointments
  • Non-payment of outstanding balances

In such cases, we will provide 30 days of emergency care while you secure a new provider.
